Oral cavity: NSF Choana: clear Glottis: clear Beak: normal occlusion Cere/nares: wnl Eyes: OD wnl; OS there is hemorrhage in the posterior segment - cannot see pecten or retina, blepharospasm; did track intermittently, but was not consistent, has normal PLRs OU Ears: dried blood around L ear, AD wnl Crop: empty Feather quality: adequate Integument: NSF Wings: NSF Wings - Range of motion: wnl Wings - Patagium: wnl Legs/Feet: NSF Legs/Feet - Range of motion: wnl Legs/Feet - Plantar surface: wnl Legs/Feet- Talons: wnl Neurologic: nystagmus in cage, not observed with bird restrained Ceolom: wnl BCS: 3 Vent: DNE Droppings: DNE Uropygial gland: DNE Other observations: painful when palpating around L eye, concern for skull fracture at orbital ridge Problem list: -head trauma --nystagmus --quiet mentation --possible skull fracture --OS trauma --AS trauma -decreased perfusion+/-hydration Plan: -Tx- gave LRS 12ml SQ will add buprenorphine -Dx- plan to repeat FS tomorrow, needs imaging Prognosis: Very guarded to poor - concerns for TBI and vision impairment AMS

Could not see well due to hemorrhage. Ears: blood in left ear Neurologic: head unstable, drifts to right and back BCS: 3 Problem list: Head trauma, blood in left ear and OS. Irregular PLRs in both eyes. Very dull. Assessment: Done by RE Plan: Tx: 15ml LRS; 0.1ml meloxicam (injectible); Ketorolac OD, NPG OS Prognosis: Poor
